Person Centred Planning (PCP)

In person centred planning the process, as well as the product, is owned and controlled by the person (and sometimes their closest family and friends). As there are no prescribed forms, tick boxes or checklists, the resulting plan of support is totally individual. It creates a comprehensive portrait of who the person is and what they want to do with their life and brings together all of the people who are important to the person including family, friends, neighbours, support workers and other professionals involved in their lives. Developing a person centred plan requires skills, values and knowledge assessment, communication, facilitation and partnership working.

This two day course assumes that participants have the skills and knowledge to apply person centred planning on a day to day basis (see An Introduction to Person Centred Care) but wish to develop advanced skills and knowledge to work with individuals with complex needs in order to facilitate person centred planning.

The course is usually offered in-house so that teams of staff can learn together how to implement this empowering approach to planning with particular service user(s) whose needs are challenging the service.
